Starting speech therapy should feel simple. Here is how the first steps work at Talk Time.
1. Get in touch
Call 917-355-6803 or send an inquiry through the contact page with your name and the best way to reach you.
2. Receive the new patient packet
The intake packet is sent to you to complete before the first visit.
3. Return your completed forms
Bring the completed packet to the first appointment, or return it as arranged with the office. Please do not email completed forms through this website.
What the intake packet includes
- Client history form
- New patient consent to the use and disclosure of health information
- Patient service agreement
- HIPAA authorization
- Authorization to release records
- Permission to communicate with your child's school or other therapists
- Photo and video permission
You can complete the packet online and print it, or call 917-355-6803 to have it sent to you. Completed forms include private health information — please return them in person, by fax to 305-675-3623, or as arranged with the office rather than through this website.
